We are currently welcoming new patients!
Our new patient visit includes an examination and treatment and may last between 30-60 minutes depending on the complexity of the evaluation.
Please complete the new patient paperwork below prior to your appointment or arrive 10 minutes early to complete the paperwork in office.
MINOR CONSENT FORM (under 18 years only)
We need this paperwork completed even if you are an established patient with Dr. Adams. Please bring this packet, your insurance card and identification to your initial visit.
If you are planning to receive acupuncture, it is helpful if you wear somewhat loose, comfortable clothing so that Dr. Kadakia can access your arms to the elbow and legs to the knee.
If you would like us to bill your insurance please check your benefits prior to your appointment. Below is an insurance verification form. This is an outline of questions to be used when calling your insurance company. It addresses most services we perform and by completing it we will be able to more accurately advise you of any out of pocket costs. We are not able to verify your benefits in office at this time.
